Hart Health & Wellness Institute, LLC (“Hart Health & Wellness Institute,” “we,” “our,” or “us”) is committed to protecting the privacy and confidentiality of your health information. This Notice describes how we may use and disclose your protected health information (“PHI”), your rights regarding your health information, and our responsibilities under applicable federal and state privacy laws.

We may use and disclose your health information as necessary for payment-related activities.

Although Hart Health & Wellness Institute operates primarily as a cash-pay practice, certain information may still be used for billing, payment processing, HSA/FSA documentation, collections, or other authorized payment activities.

Healthcare Operations

We may use and disclose your health information for healthcare operations, including quality assessment, staff training, compliance activities, auditing, credentialing, and other activities necessary to operate our practice.

Appointment Reminders and Communications

We may contact you regarding appointments, follow-up care, laboratory results, treatment information, or other healthcare-related matters.

Business Associates

We may share protected health information with third-party service providers who perform services on our behalf when permitted by law. When required, these organizations are obligated to appropriately safeguard your information.

When Required by Law

We may use or disclose your health information when required or permitted by federal, state, or local law.

You have certain rights regarding your protected health information.

Right to Access Your Health Information

You may request to inspect or obtain a copy of your medical and billing records and other health information we maintain about you, subject to certain exceptions provided by law.

We will provide access or copies within the time required by applicable law. A reasonable, cost-based fee may apply when permitted.

Right to Request a Correction

If you believe information in your medical record is incorrect or incomplete, you may request that we amend or correct it.

We may deny your request in certain circumstances, but we will explain the reason for the denial in writing when required.

Right to an Accounting of Disclosures

You may request a list of certain disclosures we have made of your protected health information during the period permitted by law.

This accounting does not include certain disclosures, such as those made for treatment, payment, or healthcare operations.

Right to Request Restrictions

You may ask us to limit how we use or disclose certain health information.

We are not required to agree to every requested restriction except when otherwise required by law.

If you pay for a healthcare service or item in full out-of-pocket and request that we not disclose information about that service or item to a health plan for payment or healthcare operations purposes, we will honor the request when required by law.

Right to Request Confidential Communications

You may request that we communicate with you about your healthcare in a particular way or at a particular location.

For example, you may request that we contact you at a specific telephone number or address. We will accommodate reasonable requests.

Right to Receive a Paper Copy

You may request a paper copy of this Notice at any time, even if you have agreed to receive it electronically.

Changes to This Notice

We reserve the right to change this Notice and our privacy practices as permitted by law.

Any revised Notice may apply to health information we already maintain as well as information we receive in the future.

The current Notice will be available at Hart Health & Wellness Institute and on our website.
